As Medical Director of Cardiovascular Rehabilitation, he saw firsthand how structured coaching, remote monitoring, and proactive outreach transformed patient outcomes. He brokered the founding partnership for a first-of-its-kind combined remote care program — and then scaled it across the division.

The SilverHeart Story

A Cardiologist Who Left a Good Thing to Build a Better One

Dr. Adam Silver grew up on Chicago's North Shore and built his early career at the same hospital that welcomed him into the world. He was surrounded by talented colleagues, invested mentors, and a community that trusted him with their care. It was the foundation any young cardiologist would be grateful for — and he was.

But five years in, a vision started to take shape that he couldn't let go of. He saw what the doctor-patient relationship could become with more time, deeper continuity, and the kind of proactive coordination that modern healthcare makes difficult at scale. Not because the people around him weren't trying — but because he believed there was a model that could go further. One where every patient leaves not with a plan, but with an itinerary. One built from the ground up around the relationship itself.

That vision became SilverHeart.

The Why Behind the Medicine

Dr. Silver didn't start in cardiology. He started with a question that had been with him since childhood: why do we lose the people we love to diseases we understand? Born with only one living grandparent, he entered medicine with a singular focus on dementia — determined to protect the grandparent-grandchild bond he never fully had.

Cardiology changed the plan. During training, he discovered that the doctor-patient rapport in cardiovascular care was unlike anything else in medicine — the trust patients placed in him when they were scared, the depth of the relationships he could build over time, and the realization that optimizing heart health was one of the most powerful tools for preventing the very cognitive decline he'd set out to fight.

Alongside his clinical aspirations, another dimension of Dr. Silver's career developed in parallel — one rooted in his upbringing. His father, a personal injury attorney who spent his career fighting for blue-collar workers in Chicago, instilled in him a deep curiosity around policy, systems design, and a persistent question: are the people who need care the most actually getting it? What began as instinct turned out to be a natural strength, one that made his career rewarding in ways he never expected. At Rush, he became the first cardiology fellow ever to hold a seat on the Cardiovascular Quality Executive Committee, serving as Director of KPI Development.

Proving It Could Be Done Differently

Before SilverHeart, Dr. Silver proved that better systems produce better outcomes — not in theory, but in measurable results. At Rush, he led an interdisciplinary team that improved the post-PCI cardiac rehabilitation referral rate by 8-fold. That work earned first place at the 2019 American College of Cardiology Health Policy Symposium.

As Medical Director of Cardiovascular Rehabilitation at Endeavor Health, he went further. He brokered the founding partnership for a first-of-its-kind comprehensive digital health personalized care program — designed to serve cardiac recovery patients and also function as a standalone virtual cardiac wellness platform. The program produced meaningful improvements in depression, anxiety, weight loss, blood pressure control, diabetes management, and patients' sense of involvement in their own care. He worked with hospital leadership to scale the program across the division and brought other cardiologists in as supervising physicians.

The playbook worked. The results were real. But the system still had limits. The only way to fully deliver the kind of medicine he believed in — longitudinal, coordinated, proactive, built around the patient instead of the billing code — was to build it from scratch.
